Mission: To provide medications to low income, uninsured individuals; to promote medication compliance by improving access to expensive medication; and to bridge the gap between medication assistance programs; and to reduce medication waste.
What You Need To Bring To Fill Your Prescription
In order for us to fill our prescription, we need to verify your eligibility. Please bring the following forms of eligibility proof with you:
- Valid photo ID
- Proof of Total Family Income (current pay stubs, tax return, child support, public assistance, social security unemployment, workman’s comp, veteran’s benefits, etc.)
- Proof of Residency (rent receipt with name and address, gas/light bill, telephone bill, water bill, cable bill, or proof of staying in a shelter)
*We will not be able to provide your medications until you bring in proof of eligibility.
